On or around the 15th of each month, submitted names will be drawn for a Birthday Cake or gift card. 

Please submit this form (or information which includes birthday person, birthdate, and contact number/e-mail) to Birthdays@townofschuylerny.gov or drop form off at town office by the 15th of the month.
